How do I prepare for my first appointment?

When scheduling your initial consultation at Re:Invent Health, our team will confirm your required documentation. To ensure a seamless experience, please prepare the following for your visit:

  • Valid Photo ID & Insurance Card

  • Current Medications & Supplements (Include dosages)

  • Recent Test Results (Last 6 months)

  • Required Referral Forms

  • Appointment Copay

Established patients only need to provide updates if insurance or medical history has changed.

What about paperwork for Re:Invent Health NYC appointments?

Re:Invent Health NYC requires the same documentation for virtual and in-person visits. To streamline your consultation, please complete our initial forms available below. Depending on your preference, you may download, sign, and re-upload the documents. Ensuring these forms are finalized 24 hours prior to your appointment allows our clinical team to focus entirely on your personalized longevity and health optimization plan.
